This section documents who attended the case conference, including all required team members, and captures the discussion and decisions made during the meeting. It also includes any notes from the reevaluation review, when applicable, to ensure a complete summary of the meeting process and outcomes.

Right below the student information section, you’ll see a question asking if the parent rejects FAPE (Free Appropriate Public Education) and plans to unilaterally enroll their student in a nonpublic school.
If you select “Yes”, you will be prompted to complete a Service Plan Document.
If you select “No”, it means the parent intends for their student to remain enrolled in the public school.
All meeting notes you add throughout the IEP will automatically pull into this page.
To keep things clear and organized, the best practice is to skip the notes boxes at the end of each page and enter all notes in the “General IEP Notes” box on this page. This keeps everything in one spot and avoids confusion since notes added on other pages won’t be separated by section here.
For an Annual Case Conference, make sure to remove any notes from previous IEPs so your document stays current and focused on this year’s meeting.
When revising an IEP (with or without a meeting) or completing a Manifestation Determination Conference, add a clear heading like: Revise IEP Meeting (4/2025)
Then type your meeting notes under that heading. Also add a heading for Previous Annual Meeting Notes underneath if needed.
Always include a heading that identifies the type of meeting and the date.
Underneath your heading, it’s helpful to list Meeting Participants with name and role (example: Jane Smith, General Education Teacher).
Don’t forget to add the E-Learning Statement and finish with a clear meeting summary that highlights the key decisions and action steps.
If you or any member of the case conference committee have concerns and feel that it would benefit the student to complete a reevaluation, it is important to connect with School Psychologist, SLP, or Related Service Providers (OT, PT,etc.) prior to the case conference. This time would allow you to talk through the need for a reevaluation and determine if updated testing would be appropriate.
Schools may request a reevaluation at any time. You do not have to wait for a case conference. If you feel new testing may need to be completed, talk with your school psychologist immediately.
If it is agreed that updated testing is appropriate, the school psychologist will prepare the Notice of Reevaluation for you or them to review with the parent(s)/guardian(s).
Make sure all completed forms are returned to the school psychologist as soon as possible to avoid any delays, since timelines for testing begin once parental consent is received.
If the CCC agrees that a student needs an FBA to be completed, select the option, "Information is needed to inform the student's case conference committee of the student's service needs, such as the student's need for assistive technology or a related service. (50-day timeline)."
Reevaluation is something that must be discussed at every annual IEP meeting. It is helpful to come prepared with your professional opinion and data to support whether or not the student is needing new testing completed, especially if the team feels the current eligibility may no longer be accurate or if another area of eligibility may need to be considered.
Please ensure you are discussing these things with the school psychologist prior to the case conference.
If a reevaluation is decided during a case conference, the TOR must contact the appropriate team member (i.e. Speech Therapist, School Psychologist, OT, PT,etc.) immediately after the meeting.
If the student is currently pending an evaluation, or if the meeting is to review a completed evaluation, you will select, "There is no need for reevaluation information."
If a parent requests for a reevaluation during the IEP meeting:
Do not add the request into the IEP document. Capture it in the meeting notes along with the parent's reasoning for the request.
Notify the school psychologist immediately. They will need to respond to the parent's request within 10 school days whether or not they will reevaluate the student.
The school psychologist will provide the parent and TOR with any necessary referral forms.
You can finalize the IEP. The school psychologist will handle the formal referral process separately.
Check the names of everyone who attended the meeting. Only the names you select here will show up on the final IEP document.
If someone attends who wasn’t listed on the Notice of Case Conference, you can easily add their name by clicking the plus sign in the bottom left corner or the box with an arrow icon.
